Rehabilitation Psychiatry

Overview

The Rehabilitation Team, based at St Vincent’s Hospital, Fairview, works with a population of service users with severe and enduring illness, whose complex needs cannot adequately be met at sector level (General Adult Psychiatry teams). Our goal is to support individuals in their Recovery journey.

The St Vincent’s Rehabilitation Team was established in 2006 and currently consists of a Consultant Psychiatrist, Psychiatric Registrar (NCHD), Clinical Nurse Manager II, Senior Social Worker and Senior Occupational Therapist. The team provides a multidisciplinary, holistic service to individuals and their families. Our team headquarters is based at 140 Lawrence Road, Clontarf, Dublin 3. We accept referrals from General Adult Psychiatry teams in St Vincent’s Hospital and the Mater Misericordiae Hospital of adults aged 18-65 years.

 

The Rehabilitation Team provides the following:

     -  Multidisciplinary assessment of new referrals, using standardised assessment tools.

     -  Individualised Care Plans.

     -  Social supports for individuals and their families.

     -  A Day Service, with Multidisciplinary input, based at 87 Lawrence Road, Dublin 3.

     -  Links with local vocational and training agencies, to ensure choice and access for our
        service user population (EVE Holdings, National Learning Network).

     -  Liaison with Dublin City Council and Voluntary Housing agencies.

     -  A Case Manager system to ensure ease of access to the team for each patient.

     -  Pharmacological and psychological management of severe and enduring illness.


Inpatient care is provided in St Vincent’s Hospital on St Louise’s ward (acute admissions) and
St Teresa’s and St Mary’s wards, for those requiring longer term care. High and medium support community residences provide accommodation for those unable to live independently. We aim to move individuals to lower levels of support once they have acquired the skills to do so.


Rehabilitation is based on a concept of increasing the individual’s autonomy. The Rehabilitation Team strive to be service user led and to provide individualised care for our patients and their families / carers, based on assessment and identified need. This treatment is based as much as possible in the community. We seek to enable service users to reach their full potential. We aspire to see not only an improvement in mental health but also an improvement in quality of life.
